A Pale Stalker appears human at a distance but becomes deeply unsettling upon closer inspection. Its skin is unnaturally pale and smooth, lacking the typical coloration or texture of living tissue. The eyes are dark and reflective, giving the impression of empty sockets in dim light.

The most disturbing feature is the mouth: an exaggerated grin extending far beyond natural facial proportions. The lips appear thin or stretched, exposing teeth even when the creature is not actively attacking.
